The first UK-wide IBD patient survey is about your experience of care for your Crohn’s or Colitis in the NHS. It will give healthcare professionals and NHS managers an understanding of what local services do well and what could be done better. They will also help to build the case for greater investment in Crohn’s and Colitis care across the UK. Support us to improve services in your area for people with Crohn’s and Colitis.

All of your survey responses will remain anonymous. Although the overall data may be viewed by your IBD service, specific results will not in any way be linked to you.

The email address you provide below will be used to send you a link to access your survey and responses securely. This will only be used for other communications if you agree to them.

The IBD Patient Survey will be live from 8th July 2019 to 22nd November 2019 .

Please choose the hospital where you have received the majority of your care over the past 12 months. If you have NOT recently received hospital care or are not under the care of an IBD service, please select the hospital you live nearest to. Where hospitals are working together to provide an IBD service, we have listed these together. Please let us know by emailing if you are unable to find your service from this list or if it needs updating.
This survey is only for UK residents receiving care in the UK
